Dr. Martin Luther King Jr. Middle School Athletics 2022-2023

Hello MLK Families, My name is Ms. Delfino, the Athletic Director at MLK Middle School. MLK Athletics will be moving our athletic paperwork to a digital format. We are now asking parents/guardians to use the website: Homecampus.com to complete all athletic paperwork for each school year. The forms will remain the same but will now be a digital signature. Moving to Home Campus will allow everyone to stay updated with athletes being cleared for each sport. Below is the link to the website, instructions on how to navigate the website, and a walkthrough video.  I would encourage your child to list multiple sports the first time, otherwise they will have to submit a new account later for other sports.

*Note* if planning on playing multiple sports check off all that apply, to avoid repeating paperwork.

• Student Athlete Pre-participation Physical Form (requires doctor signature. You will upload page 4 of this form to the athlete's Home Campus account.)
• If you have already submitted paper copies of the forms you still need to create an account on homecampus.com, Ms. Delfino will then upload the athlete's physical to your account.

Athletes can participate in tryouts once all forms are completed and submitted on Home Campus.
