  • Open Enrollment is the time to add or update your medical, vision or dental plan; add or drop dependents from coverage; enroll or re-enroll in healthcare or dependent care flexible spending accounts (HSAs or FSAs); and enroll in other voluntary benefits.
  • Open enrollment for health and vision plans begins Monday, September 30, 2024, and ends on Friday, October 25, 2024, 5:00 p.m., PT. Health and vision plan elections are managed by the San Francisco Health Service System (HSS).

Below are instructions for changing certain elections during open enrollment. Between now and the next open enrollment window, no changes can be made to the below unless you have a qualifying life event.

I want to add, change, or update my health benefit elections

Enroll with eBenefits

All SFHSS Members can now make their health benefit elections online with eBenefitsan easy and secure system that walks you through each step of the enrollment process. After you review your plan documents, log into eBenefits from the City's Employee Portal.  If you are new to eBenefits, you will need to create a new account.
 

eBenefits Compatible Internet Browsers: eBenefits works best with Firefox, Microsoft Edge, and Safari browsers. Do not use Google Chrome until further notice.

 

Step 1 - Review your Open Enrollment Letter and Rates

Review your Open Enrollment Letter for current medical, and vision elections and rates.

Do you have changes you want to make?

If YES, go to Steps 2 through 5 on how to make changes.

If NO, continue to Step 2.

 

Step 2 - Add or Drop Dependents

Review the dependent(s) listed in your enclosed Open Enrollment letter and dependent eligibility rules here.

Do you need to add or drop a dependent? 

If NO, and you have no changes to your benefit elections, then you have no further actions to take.

If YES, complete the Review Dependents page in eBenefits to add dependents or modify existing dependents.

Save and continue through all the screens and confirm at the end to submit your changes.

Submit copies of supporting documents. New dependents must have supporting documentation submitted with their elections in order to be enrolled (e.g. birth certificate, certified marriage certificate).

 

Step 3 - Making Changes to Your Health Plan Benefits

Start by reviewing the Service Areas of the medical plans available to you here. There are five (5) medical plans to choose from, including four (4) HMO plans and one (1) PPO plan. Review your options below. You can compare in-network providers by HMO plans on page 11, and a summary of benefits for all medical plans.

Step 4 - Vision Benefits

Review the Vision benefits options here. Rates can be found on the back of your Open Enrollment letter or on page 18 of the digital 2025 SFUSD Benefits Guide after Step 6 on this page. You must be enrolled in a medical plan to receive basic vision benefits.

Enrollment in the VSP Premier Plan requires that all dependents enrolled in medical coverage be enrolled in the VSP Premier Plan.

Step 5 - Complete Your eBenefits Elections Online

Complete your eBenefits elections online. Click here for instructions on navigating eBenefits.

Be sure to click Save and Continue through each screen. You must click Submit at the end in order to complete your enrollment. Otherwise your elections will not be recorded.

Step 6 - Confirmation Letter

After Open Enrollment ends, you’ll receive a Confirmation Statement in the mail in December. Please review your Confirmation Statement to make sure your benefit elections are correct. Changes made during Open Enrollment take effect January 1, 2025.

How to Reset Your Password

1. Go to https://myapps.sfgov.org/ccsfportal/forgotPassword.   

2. Enter Your DSW Number or User Name.

3. You will get to password reset where you use your previous MFA info. to get back in.

I want to contribute to a healthcare or dependent care FSA

If you want to contribute to a healthcare or dependent care FSA, you must log into your WageWorks profile and make elections for the 2025 calendar year. FSA elections do not automatically renew every year. Please click the links below for instructions on creating a WageWorks account and making elections.

I want to add a dependent to my dental coverage

To add a dependent to your dental coverage, you must first add your dependent using the instructions located in Enroll Dependents into Benefit Plans. You must upload supporting documentation (marriage or domestic partner certificate and/or children’s birth certificate). Please note that when you add a dependent, a notification will be sent to the Benefits team, as they must approve any changes made to your benefits profile. You will not be able to add dependents or beneficiaries to your benefit plan until the dependents have been approved.

Once the benefits team has approved the dependent(s), the dependents section will update to reflect your dependent’s information. This means that you will now be able to add them as a beneficiary or a dependent to your dental plan.
