It is imperative that each student has a current student emergency card on file at the school of attendance. Emergency cards can be completed one of two ways: A hard copy will be distributed to students on the first day of school and must be returned promptly; or, families can complete an online emergency card in ParentVue. Make sure your address and phone number(s) are current so school staff contact the parent/guardian/caregiver or designee immediately in case of an emergency. The student’s health care provider and insurance information also should be current in case of an emergency.

Any medication(s) needed during the school day, as well as chronic illness(es), should be noted on the card so appropriate action can be taken in case of an emergency. This information will also prompt the school to reach out to parents/guardians to obtain necessary medication and care plan authorizations.
