The Ages and Stages Questionnaire is an assessment for Pre-Kindergarten Students (Infant/Toddler/3 and 4 year olds) that is given to parents to complete and return to classroom teachers.

The ASQ screening helps families and teachers work together to focus on children's strengths and weaknesses for specific skills. They are able to identify more closely where children may need help with certain skills over the other.

How are specific ASQ assessments assigned to students? Link to this section
The ASQ Calculator can be used to check which ASQ is appropriate for each student according to their age on the date of administration.
Enter in the student’s info that is required under the ASQ Age Calculator (enter in “0 weeks” for weeks premature) and the calculator will calculate the correct assessment in months for you.
How do I view ASQ results? Link to this section
The ASQ 2024-25 Report can be accessed via Dna (Illuminate) after the close of each administration in Fall, Winter, and Spring.
Look for the letter Y in columns beneath each domain header to identify students with additional needs after this screening.
