This form is for SFUSD families to request to borrow technology for their students or to request an exchange for an SFUSD device that is no longer working. Students attending SFUSD schools in grades PK-12+ can request a Chromebook computer and/or Hotspot for Internet access if they do not have access to a computer or Internet access that can be used every day for distance learning.
