The Health Education Accountability Tool (HEAT survey) is designed to record the 20 or more health lessons you have taught during the 21-22 school year. This survey data will help Student & Family Services track health education across the district in order to align services and resources. Use the link below to complete the survey, which should take 5-10 minutes.

• Please also report health lessons delivered to your students by other staff, including the librarian, school social worker/nurse, physical education, or garden teacher. Include the lessons you plan to teach through the end of the year.

• Reminder:  SFUSD Board policy mandates a minimum of 20 lessons of health education.  SFUSD Board of Education Policy #19-245pl
