Changes to Smartsheets Case Submission Form: 12/13/21
Beginning December 13th, two questions will be added to the Smartsheets Case Submission Form when positive cases arrive.
When you indicate a staff or student has tested positive, you will be asked for information on the type of the positive test (Color test, rapid test, etc.), and if the index case has children or siblings in SFUSD. These questions will help with the contact tracing and notification process.
