Edit Transcript/Course History google form This can be used if teachers need to make a grade change to a semester final grade, this is not for quarter grade changes.
Please make a copy of this form and enter your school name and principal on the form prior to sharing it with teachers.
Grade changes require a teacher signature so teachers must use their SFUSD email which will count as their signature.
It is also required for sites to maintain a record of all the grade changes made at the school site.
