Prek, Kindergarten and 7th-grade Immunization Compliance Reports must be submitted on www.shotsforschool.org by January 29, 2021.

The following staff are aware that they are responsible for submitting the Immunization Compliance Report:

• PreK/Kindergarten: health worker
• 7th Grade: school district nurse

Please check in with your health worker/school district nurse to see how you can support their efforts. It may include:

• an immunization compliance reminder in a family newsletter
• a discussion at a CCT on how staff can help outreach to families  

Our health workers and school district nurses work diligently to ensure your school is in compliance with the California Health and Safety Code Section 120375 and California Code of Regulation Section 6075. Your support is appreciated!

DPH will be sending email reminders to principals regarding this upcoming deadline. During distance learning, no student will be excluded from attending classes based on their immunization compliance status.
