SF Department of Public Health (SFDPH) Community Health Programs for Youth (CHPY) Physicals Announcement

Effective October 15, 2024: 

CHPY is pleased to announce an important update regarding our sports physical services. We are now offering sports physicals for patients who are uninsured or covered by Medi-Cal.

Who Can Receive Sports Physicals at CHPY?

  • Uninsured Patients
  • Medi-Cal Patients

Important Information for Privately Insured Patients:

If you have private insurance, please note that you will need to obtain your sports physical from your primary care physician.
